Service Users (Bereavement support service)
7.1 If you are receiving a service from us, we will collect your data as part of you receiving that service. This will include quite sensitive information relating to your emotional health and well-being, relevant to the kind of support that we would be providing for you.
7.2 Before engaging or receiving services from us we will have explained all this relevant information to you and asked your specific permission/consent gained to be able to gather, hold and process this information for a defined period of time.
7.3 If you are under 13 we would need consent of a relevant adults to hold your personal information.
7.4 The children’s and young peoples’ personal data is gained from the person who has parental guardianship for them. This may include their name, address, telephone number, email address, gender, date of birth, name of your parent(s) and their contact information, if they are adopted/fostered, ethnic origin/race, the nature of your loss event (s) and their dates, relationship to the person stated in the loss event, name of their school, any food allergies/likes or dislikes, any specific special needs or disabilities they may have and any emotional difficulties they may be experiencing as indicated in their registration form.
7.5 We use the above data from the children and young people we are supporting for the sole purpose of providing the best service and support that we can. We will always evaluate the service we provide by way of service evaluation from the service users (children/young people) and their parents as this helps to evaluate the quality of service we provide and the ability to audit our practice and its effectiveness.
Volunteers and training of Site Users
7.6 We may process your information included in your personal profile on our website. The profile data may include your name, address, telephone number, email address, gender, date of birth, relationship status, ethnic origin/race, church address/community group and religion or the nature of your loss event. The profile data may be processed for the purposes of enabling and monitoring your use of our website and services. The source of the service data is you or your church/community group. 7.11 As part of the above customer relationship mentioned in 7.10, each group/site will be registered on a general database of all operational groups within the UK and held by UK representative, Mary Gault. She will contact you periodically as part of this ongoing relationship to ascertain if your local group/site is still active and operational and if you want it to be still wish to remain registered. 7.12 We may process information relating to transactions, including purchases of goods and services, that you enter into with us and/or through our website (“transaction data“). The transaction data may include your contact details, your card details and the transaction details.
